Our allergy providers and staff look forward to meeting you and your child for a comprehensive allergy evaluation.
We welcome all patients from infancy through 25 years of age, accepting patients not only from our Branford Pediatrics & Allergy practice, but from all outside practices as well. Patients may be either self-referred or referred by a medical provider.
Our practice is committed to providing the highest quality of care for our patients, offering personalized attention in a friendly, courteous environment.
Welcome Letter and Patient Instructions
Prior to your visit, please read the Welcome Letter for information regarding your visit and the allergy testing you might receive. This letter contains special instructions on how you must prepare for your visit.
New Patient Forms
PLEASE PRINT THE 3 FORMS BELOW AND FILL THEM OUT COMPLETELY. Bringing these forms with you to the appointment will save you time in the waiting room.
- The Allergy History Form is important information for evaluation and treatment. It is helpful to take your time filling out this information at home, rather than feeling rushed at the visit.
- The Demographic Form gives us up-to-date information and outlines the members of your family.
- The Authorized Representative Form tells us the people you allow us to communicate with regarding your child's care. IMPORTANT: This form does not permit the persons listed to consent to any care your child receives or to ask for your child’s records; it simply allows us to talk with them about your child’s care.
Please make sure all insurance referrals are completed before your visit with us. Our Insurance page lists our participating insurance companies.
We are happy to reschedule your appointment if necessary. If you are unable to keep your appointment, we request that you call our office to cancel within 24 hours of the scheduled appointment.
